Audit a sample of meeting recaps posted from Teams meetings: [RECAP_SAMPLE]. Whether actions were completed, if known: [ACTION_OUTCOMES].

Describe how you counted: a recap has an owner only if every action names a person, and a date only if every action has one. Report the three counts (owners, dates, decisions stated) separately and combine them into a score out of three per recap, along with the share of actions completed and the average days to publish, with arithmetic and the sample size. Identify the best recap and what makes it good, so a template can copy it. Look at clarity for people who were not there and at follow-up, without judging individuals, and remember that the sample may not represent all meetings.

Propose a template that fixes the gaps, a rule to publish within a day, and a ten-minute end-of-meeting habit in which the note-taker reads the actions aloud and gets confirmation, showing how it would change the numbers. Provide a sample recap following the template and a check at the next meeting.
