Include a "top five lessons" page, a revised starter register for similar projects, and five questions for the team. Keep the tone blameless. Do not invent outcomes. Finish with the single risk category that most deserves a better checklist next time, and say why in one sentence. List which risks on my register were never discussed after the first month, since silence often hides drift.
