Hallucination Reduction Checklist for a Prompt
Tighten a prompt to reduce invented content.

Go through this checklist, marking Pass, Fix, or Unknown with a quote from my prompt and a fix:
[ ] The prompt supplies the facts the answer depends on, instead of expecting the model to know them
[ ] It says to use only the provided material for factual claims
[ ] It tells the model what to do when the information is missing ("write 'not stated' and ask a question")
[ ] It requires quotes or references for claims, and bans invented references and links
[ ] It does not ask for precise facts the model is unlikely to know (obscure statistics, recent events, exact legal citations)
[ ] It separates facts from opinions or suggestions
[ ] It limits scope: one task at a time
[ ] It asks for a confidence level and a list of claims to verify
[ ] It avoids leading questions that assume false premises
