Audit our decision log. Decision records: [DECISION_RECORDS]. Decisions I know were made in the period but may not be recorded: [KNOWN_DECISIONS]. Our decision template: [TEMPLATE].

For each record check that the decision is stated clearly and that options, reasons, owner, date, review date and status are recorded. Count a record as complete only when every template field has real content, not a placeholder. Count the share of known decisions that have a record, and identify records missing reasons or options, since these will confuse later readers.

Show a short table first, then the explanation. List known decisions without a record in order of how much they affect other teams, with who is best placed to write each. Suggest a trigger for what kind of decision must always be recorded, with three examples from my data, and edits to the template if it does not ask for what you found missing. Do not invent reasons for old decisions; mark them for the decision maker to complete.
