Audit a sample of meeting notes pages. Pages: [NOTES_SAMPLE]. Whether actions were completed: [ACTION_OUTCOMES].

Define your terms before counting: what you treated as a decision, an action and an owner. If a page mixes them, count it once and note the problem. Score each page for clear decisions, actions with named owners and dates, a summary at the top, time to publish after the meeting, and whether previous actions were reviewed. Calculate the share of actions with an owner, with a due date and completed, with the sample size beside every percentage, and identify the meeting types with the lowest follow-through. Small samples can mislead, so avoid conclusions a handful of pages cannot support.

Look at the habits of the meeting, not individuals. Return a table, three patterns with one small experiment each (for example a rule that actions are agreed before the meeting ends), a template that fixes the three most common gaps, and a two-sentence message to introduce it to the team.

Invented sample of 12 notes pages. Decisions stated: 9 (75 percent). Actions with owner: 8 (66.7 percent). Actions with date: 5 (41.7 percent). Published within a day: 7 (58.3 percent). Previous actions reviewed: 4 (33.3 percent). Of 31 actions recorded, 19 were completed (61.3 percent).
Pattern: meetings that review earlier actions complete more (3 of 4 meetings versus 3 of 8). Experiment: add a 'last time' line to the template and agree owner and date aloud before closing. Method note: the sample is 12 pages, so treat the percentages as a direction, and recount after the next four meetings to see whether completion rises above 70 percent.
