Write a usability test report. Study goals, method, participants: [STUDY_DETAILS]. Findings and observations: [FINDINGS]. Metrics: [METRICS]. Audience: [AUDIENCE]. Produce: executive summary (150 words: what was tested, headline result, top three problems, recommended next steps); method and participants (brief, with limits); task results table (success, time, errors, satisfaction) with the arithmetic for success rates; findings ordered by severity, each with evidence (quotes, counts), an annotated description, and a recommendation; positive findings; open questions; and next steps with owners. Rules: findings stated plainly, one issue per finding, distinguish observation from interpretation, avoid blaming participants, and protect identities (use IDs). Add guidance on a 10-minute readout and a one-page poster format. Include a note on what not to conclude from a small sample. Add a table of task results with success rate, median time, and errors, the arithmetic for each success rate, and a final recommendation list with owners. Keep the main report to four pages and move detail to an appendix.
