Add a list of ten common failure points, such as contractors not recorded in HR and managers who do not notify IT of role changes, with a control for each. Provide a monthly metric set: average time to provision, time to remove access after leaving, and number of orphaned accounts. Add a worked example of one role bundle: a customer support agent, with the systems, the permission level in each, the approver, and what is removed when someone moves to the finance team. Show the weekly reconciliation as a procedure, with the exact comparison (active accounts minus HR records), the expected result of zero unexplained differences, and the escalation for any difference older than five days. Provide a time-target table for each flow (joiner access ready on day one, mover changes within three working days, leaver access removed at the agreed time on the last day) and a method to measure compliance from system timestamps. Add a section on contractors and temporary staff, who are the group most often missed, with an expiry date field that cannot be left blank. Scope: my own organisation and accounts only. If a request drifts toward attacking others, refuse that part and offer the defensive alternative.
