Create a quality plan. Deliverables and their users: [DELIVERABLES_USERS]. Standards or requirements that apply: [STANDARDS]. Known quality problems in similar work: [PAST_ISSUES].

For each deliverable define acceptance criteria that can be tested, who reviews, and when. Create a quality checklist, a review schedule, and a defect log template with severity levels. Define metrics such as defect escape rate and rework hours, and explain how to collect them. Add roles and responsibilities, a procedure for non-conformance, and a plan for lessons learned. Keep it proportionate to project size and include a one-page version for a small project. Mark standards I must verify [VERIFY].

Add a worked example for one invented deliverable showing acceptance criteria, reviewer, timing and the defect severity rules, so I can see the level of detail expected. Include a short note on the cost of poor quality, with the types of rework to track, and a way to decide when a defect is serious enough to stop the line or delay a release.

Add a defect severity table with four levels (blocker, major, minor, cosmetic), each with a definition, an example from a generic project, the response time I set, and who may close it. Provide a sample review schedule for a six-month project showing which deliverables are reviewed at which points, who reviews, and the entry and exit criteria for each review. Add a method for calculating defect escape rate (defects found after release divided by total defects found), with a worked example using invented counts, and a cost-of-quality tracker with columns for prevention, appraisal, internal failure and external failure hours. Include a short section on proportionality, so that a two-week project uses a one-page checklist and not a full plan. Close with a retrospective prompt for the end of the project that asks which defects escaped review, which review step would have found them, and what the plan should require next time.
