Create and analyze an onboarding survey. Company size and onboarding stages: [COMPANY_AND_STAGES]. Responses (if available): [SURVEY_RESPONSES]. Goals: [GOALS].
Part 1: write a survey of 12 questions for day 30, mixing a 1 to 5 scale ("I understood what was expected of me in my first week") and open questions ("What one thing would have helped most?"). Keep it under 5 minutes, neutral wording, and anonymous or confidential with a clear statement of who sees results.
Part 2: if responses are provided, compute the average score per question, the share of low scores (1 or 2), and rank questions. Group open comments into themes with counts and a quote for each. Say plainly when the sample is too small for firm conclusions.
Part 3: produce an action plan table: Finding | Evidence | Fix | Owner | Date | Metric. Add a template for sharing results with the team and a rule to tell new hires what changed because of their feedback. Do not invent responses.
