Design a decision log and meeting index in Notion for my team. Team size and the kinds of decisions we make (product, hiring, spending, process): [TEAM_AND_DECISIONS]. People who should be able to see them: [AUDIENCE]. Meetings we hold: [MEETINGS].

Specify two linked databases. Decisions: title, date, decision (one sentence), reasons, options considered, owner, people consulted, status (proposed, decided, revisited, reversed), review date, and link to the meeting or project. Meetings: title, date, type, attendees, notes link, and decisions made (relation). Views: recent decisions, by owner, by project, due for review, and a meeting calendar. Page template headings: Context, Options, Decision, Consequences, Review.

Worked example (invented): choose supplier B for the pilot, decided on the 3rd, review on the 3rd of next month, with two options and reasons. Rules: write the decision within one day, record disagreement honestly, and never edit a past decision; add a new entry that supersedes it. Quarterly review of decisions that need revisiting. Mark property types and view features [VERIFY].
