Community Event Feedback Survey and Impact Report
Design a short post-event feedback survey and a one-page impact report for funders, volunteers and the community.
The Open buttons send the prompt text as currently shown below to that site only when you click. Fill in the placeholders first.
Help me learn from a community event I ran. Event name and type: [EVENT_NAME_AND_TYPE]. Date: [EVENT_DATE]. Number of attendees and volunteers: [ATTENDANCE_COUNTS]. What we were trying to achieve: [EVENT_GOALS]. Who will read the report: [REPORT_AUDIENCE]. Data I already have (sign-in sheet, photos, comments, costs): [AVAILABLE_DATA]. First, write a survey of no more than 8 questions that takes under 3 minutes: a mix of a 1 to 5 rating, two multiple-choice items, and two open questions, with wording that avoids leading questions. Provide a version for a paper card and one for online use, and a short consent line about how the answers will be used [VERIFY privacy rules for my area]. Second, give a one-page impact report template with the headings Headline numbers, What went well, What we would change, Quotes (only real ones I supply), Money in and out, Thank-yous, and Next steps. Show how to calculate response rate, average rating and cost per attendee, with the arithmetic and a note that low response rates limit conclusions. Third, list five decisions the feedback could change next time. Do not invent quotes, results or numbers; leave placeholders. Do not draw strong conclusions from tiny samples; say so plainly. Add a worked example with invented figures: 150 attendees, 45 responses, ratings summing to 189, total cost 1,800, and compute response rate, average rating and cost per attendee.

Invented figures, worked through. Response rate = 45 / 150 = 0.30, so 30 percent. Average rating = 189 / 45 = 4.2 out of 5. Cost per attendee = 1,800 / 150 = 12. Reading: 45 responses is a modest sample and people who enjoyed the day are more likely to reply, so describe 4.2 as "among those who responded". Headline numbers: 150 attendees, 22 volunteers, 4.2 average rating, 30 percent response rate. What we would change: the two most common open answers mentioned queues at registration. Next step: add a second sign-in table and test it at the next planning meeting.
Illustrative only, made-up data. Your results will differ and must be reviewed.
