Classify errors, find the stage where they enter, calculate the error rate and the cost per error from my figures, and identify process changes such as barcode scanning or labelled bins. Also suggest checks at the point of packing, such as scanning items against the order, and a daily sample inspection by a second person. Add a record of errors by shift and product, a short debrief with the team that is blame-free, and a target error rate set from my own history. Then design a pilot to reduce errors and a method to prove whether it worked. Choose one change, such as scanning at packing or a second check for high-value orders, and define the pilot period, the orders included, and the measure of success. Calculate the cost of the change against the cost of errors using my figures. Write a short procedure for the pilot and a briefing for staff that frames it as improving the system. Create a daily error log with fields for order, type, stage found, cause and cost, and a weekly ten-minute review meeting agenda. Explain how to compare the pilot with a baseline period fairly, taking account of busy days. Add a plan for scaling or stopping and for sharing results with the team, with recognition for suggestions that improve the process. Add a final item: ask the people who pick and pack what makes errors likely, and list their answers verbatim before any solution is chosen. Add a closing section on lessons for similar work in future, in four bullet points that a team could pin above a desk and apply without needing the full report.
